Support Coordination in the Redlands: Connecting with Local NDIS Services

The Redlands includes the bay islands, and that changes what support coordination has to do. This guide explains how coordination works on the mainland and the islands, and what to ask a Redlands coordinator.

Support Coordination in the Redlands

Redland City is a mainland strip from Thorneside to Redland Bay plus a set of islands reached by ferry and barge, and NDIS coordination has to work on both. The mainland is reasonably well served; the islands are not, and a coordinator who cannot get providers across the water is not much use to an island participant. MediHealth Connect provides support coordination in the Redlands, including the islands, and this guide covers what local coordination involves.

Coordination on the Mainland

Cleveland, Capalaba, Victoria Point and the suburbs between them have a reasonable spread of providers, with Redland Hospital at Cleveland as the public hospital and specialists mostly in Brisbane. Coordination here looks much like anywhere else: knowing who has capacity, who covers which suburbs and how Redland Hospital and Metro South handle discharge.

Coordination on the Islands

Russell, Macleay, Coochiemudlo, Lamb, Karragarra and North Stradbroke Island are a different problem. Some providers will not travel; some will, at a cost that has to be funded; a few have staff who live on the islands. A Redlands coordinator keeps a current list of who will cross the water, plans visits around the ferry timetable, helps participants use provider travel funding within the NDIS rules and looks at telehealth where a support cannot be delivered in person. Ask any coordinator how many island participants they currently support.

Redland Hospital and Discharge

For participants leaving Redland Hospital, a coordinator involved before the discharge date arranges nursing, equipment and support workers and, for island participants, the ferry home and the delivery of consumables. Where the plan does not fund what is now needed, the coordinator requests an urgent plan review. Our guide to NDIS nursing after hospital discharge covers the clinical handover.

Choosing a Coordinator in the Redlands

Check registration for your funded level. Ask whether the coordinator visits island participants in person and how often. Ask about caseload and leave cover. Ask how the provider handles conflict of interest if it delivers other services; MediHealth Connect also provides community nursing in the Redlands and answers with a written process. Ask how reviews are prepared and when preparation starts.

Transitions in the Redlands

Moving between the islands and the mainland, school leavers, hospital discharges and carers who can no longer manage are the transitions Redlands coordinators handle most. Moving to independent living on an island raises a hard question about which supports can actually be delivered there, and a coordinator answers it honestly before anyone signs a lease. Our life stage and transition support page explains more.

Frequently Asked Questions

Do you coordinate supports on the Southern Moreton Bay Islands?

Yes. Russell, Macleay, Coochiemudlo and North Stradbroke Island participants are part of our Redlands service, with visits planned around the ferry timetable.

Can provider travel to the islands be funded?

Within the NDIS rules, provider travel can be claimed, and a coordinator helps you understand what your plan will cover and when telehealth is the better option.

How quickly can coordination start in the Redlands?

A first meeting within days to a couple of weeks, on the mainland or the island, with services arranged over the following weeks depending on which providers will travel to you.
