MediHealth Connect implements positive behaviour support in Moreton Bay, from the Redcliffe Peninsula to Caboolture, North Lakes and Bribie Island. Our support workers are trained on your behaviour support plan, stay consistent, and give your practitioner the information they need to keep the plan working.
Positive behaviour support is a way of understanding and responding to behaviours of concern that puts the person first. Instead of asking how to stop a behaviour, it asks what the behaviour is achieving for the person and what could be changed so they no longer need it. The answers are usually practical: a different routine, clearer communication, a health issue treated, a skill taught, an environment made calmer.
In the NDIS, positive behaviour support is split between a registered behaviour support practitioner, who assesses the person and writes the behaviour support plan, and the implementing providers who put that plan into practice every day. MediHealth Connect is an implementing provider in Moreton Bay. Our role is to follow the plan faithfully, keep the team steady and report back to the practitioner.

Every worker is trained on your plan before working with you. That covers the preventive strategies, your particular early warning signs, the responses your practitioner has approved and the things that must never be done. We attend implementation training when your practitioner offers it.
We keep your team small and local. A steady group of workers who know your routine reduces the unpredictability that so often drives behaviours of concern, and it means someone notices when things start to shift.
Workers record incidents and near-misses, including what came before, how they responded and what happened afterwards, in the format your practitioner requires. That data is passed to your practitioner on a regular basis.
Your practitioner leads and we follow. We bring problems to them rather than improvising, and when a plan is reviewed we retrain the team on the new version before it is used.
MediHealth Connect does not write behaviour support plans and does not carry out functional behaviour assessments. Under NDIS rules that work is reserved for behaviour support practitioners registered with the NDIS Commission for specialist behaviour support, and we are not registered in that group.

Where a behaviour support plan includes a regulated restrictive practice, strict safeguards apply, and Moreton Bay families should know what they are.
Queensland law requires that a regulated restrictive practice is authorised before it is used with an NDIS participant, and the practice must be written into a behaviour support plan lodged with the NDIS Commission. The plan must show that less restrictive strategies were considered, and the practice may only be used in the least restrictive way for the shortest time. The implementing provider must record every use and report to the NDIS Commission, and any unauthorised use or use outside the plan is a reportable incident.

No. Behaviour support plans and functional behaviour assessments must be prepared by a behaviour support practitioner registered with the NDIS Commission for specialist behaviour support. MediHealth Connect is not registered for that service. We implement the plan, and our Moreton Bay support coordinators can help you find a practitioner.
Every worker on your Moreton Bay team is trained on the full plan before they support you, including proactive strategies, early warning signs, approved responses and excluded actions. We attend your practitioner's implementation training where offered and retrain the team whenever the plan changes.
It is used only as authorised under Queensland law and as written in the plan lodged with the NDIS Commission. We record each use, report to the Commission as required and report any unauthorised use as a reportable incident. The plan is always aimed at reducing and removing the practice.
Yes. We follow their direction, share data with them and raise issues with them directly. If you change practitioners, we support the handover so your implementing team does not change.
The practitioner is usually funded from Capacity Building under Improved Relationships, while the workers implementing the plan are funded from your Core budget as part of your daily supports. Your plan manager or support coordinator can confirm what your plan provides.
