Positive Behaviour Support Logan

MediHealth Connect provides positive behaviour support in Logan by implementing the behaviour support plan your practitioner has written. A consistent team of trained support workers follows the plan at home and in the community, records what your practitioner needs to know and keeps the whole approach on track.

What Positive Behaviour Support Means for NDIS Participants

Behaviours of concern rarely come from nowhere. They are usually the most effective way a person has found to deal with pain, frustration, fear, boredom or a need that is not being met. Positive behaviour support works out what the behaviour is doing for the person and then makes changes, to the environment, the routine, the way people communicate and the skills the person has, so that the behaviour is no longer needed.

Within the NDIS this happens in two stages. A registered behaviour support practitioner carries out the assessment and writes the behaviour support plan. Then the people around the participant implement that plan every day. MediHealth Connect works at the implementation stage in Logan, supplying support workers and nurses who have been trained on the specific plan they are carrying out.

How We Implement Behaviour Support Plans in Logan

In our experience across Logan, plans break down not because they are badly written but because the people delivering them have not read them, keep changing, or record nothing. Our service is designed around fixing those three things.

Training on Your Plan

No worker starts with you until they have been taken through your plan in detail: proactive strategies, the early signs that things are building, the responses your practitioner has agreed and anything that is off limits. We attend practitioner-led implementation training whenever it is available.

Consistent Staff

We build a small core team who know you and stick with you. Unfamiliar workers are a common trigger, and consistency is often the single most effective strategy in any plan.

Data Collection

Our workers record behaviours, triggers, responses and outcomes in the format your practitioner specifies. That data is shared with your practitioner on a regular schedule so plan reviews are based on facts.

Working With Your Practitioner

We follow the practitioner's lead, flag anything that is not working, and never adjust strategies on our own. When the plan is updated, every worker is retrained before the new version goes into use.

Who Writes Your Behaviour Support Plan

We want Logan families to have no doubt about this: MediHealth Connect does not write behaviour support plans and does not carry out functional behaviour assessments. The NDIS requires those to be done by a behaviour support practitioner registered with the NDIS Commission for specialist behaviour support, and we are not registered in that category.

If you need a practitioner, our Logan support coordinators can help you find one who is accepting new participants, work out how to use the Improved Relationships funding in your plan and connect the practitioner with the implementing team. Once your plan is written, we put it into action.

Regulated Restrictive Practices and Reporting

If your behaviour support plan includes a regulated restrictive practice, such as a locked gate, a medication used to manage behaviour, or a physical hold, there are rules that every implementing provider in Logan must follow.

Queensland law requires authorisation before a regulated restrictive practice can be used with an NDIS participant, and the practice must be set out in a behaviour support plan lodged with the NDIS Commission. The plan must explain why less restrictive alternatives are not enough, and the practice must be applied in the least restrictive way for the shortest time. The implementing provider must record every use and report it to the NDIS Commission, and any use that is unauthorised or outside the plan is a reportable incident.

Our Logan team meets those obligations in full and works with your practitioner towards phasing the practice out.

Positive Behaviour Support Logan: Frequently Asked Questions

Do you write behaviour support plans?

No. Writing a behaviour support plan and carrying out a functional behaviour assessment are jobs for a behaviour support practitioner registered with the NDIS Commission for specialist behaviour support, and MediHealth Connect is not registered for that. We implement your plan, and our Logan support coordinators can help you find a practitioner.

How do you train staff on my behaviour support plan?

Each worker on your Logan team is trained on the complete plan, covering proactive strategies, early warning signs, agreed responses and excluded actions, before they support you. We attend your practitioner's implementation training where it is offered, and we retrain the team whenever the plan is revised.

What happens if a restrictive practice is in my plan?

It is only used as authorised under Queensland law and as written in the plan lodged with the NDIS Commission. Every use is recorded and reported to the Commission, and any unauthorised use is reported as a reportable incident. The purpose of the plan is to reduce and remove the practice.

Can you work with my existing behaviour support practitioner?

Yes. We work under their direction, share data with them and raise problems with them directly. If you change practitioners, we help with the handover so the implementing team stays consistent.

How is behaviour support implementation funded?

The practitioner is usually funded from Capacity Building under Improved Relationships. The support workers implementing the plan are funded from your Core budget as part of your daily living or community supports. Your plan manager or support coordinator can confirm the details for your plan.
