Our office is at Darra in Brisbane\'s western suburbs. Most of our work happens in participants\' homes, so if you would like to meet in person, call ahead and we will arrange a time at the office or at your home.
We provide community nursing, support coordination, plan management and other NDIS supports across South East Queensland. Each regional page explains what is available locally.
1. We call you back. A member of our team phones to talk through what you need, which region you are in and what your NDIS plan funds. If you do not have your plan handy, that is fine.
2. We confirm what we can offer. Not every service is available in every suburb at every time, so we tell you plainly what we can provide and when we can start rather than promising and then apologising.
3. Assessment or first meeting. For nursing, a registered nurse visits to assess and write a care plan. For support coordination or plan management, your coordinator or plan manager meets you to go through your plan and set up a service agreement.
4. Services begin. Once the agreement is signed and funding is confirmed, your supports start on the schedule agreed with you.
If you're a support coordinator, LAC, or healthcare professional looking to refer a participant, our dedicated referral form makes it quick and easy.
We aim to respond to all enquiries within one business day. Urgent matters are prioritised.
It helps if you can share your plan details, but it's not required for an initial conversation. We can help you work out whether your plan covers our services.
Yes. Family members, carers, support coordinators, and healthcare professionals are all welcome to enquire on behalf of a participant (with their consent).
