MediHealth Connect is a registered NDIS provider based in South East Queensland, delivering community nursing care and support coordination to participants across Brisbane, Gold Coast, Logan, Ipswich, Moreton Bay, and the Redlands.

We started because we saw too many NDIS participants struggling with fragmented care, a nurse who didn't communicate with the support coordinator, a coordinator who didn't know the local providers, and services that felt transactional rather than personal. We set out to do it differently.

What We Stand For

Clinical Quality Without the Clinical Feel

Our nurses are highly qualified and clinically rigorous. But they're also warm, approachable, and respectful. We deliver hospital-grade nursing care without making your home feel like a hospital.

Coordination That Actually Coordinates

Support coordination should mean your services work together, not just that someone filed the right paperwork. Our coordinators actively manage your care network, facilitate communication between providers, and step in when things aren't going to plan.

Local Teams for Local Communities

We don't operate from a single office and dispatch staff across the state. Our nurses and coordinators are based in the communities they serve. They know the suburbs, the hospitals, the GPs, and the allied health providers in each region.

Independence as the Goal

Every interaction we have with a participant should move them towards greater independence, whether that's building skills to manage medications, learning to self-advocate at plan reviews, or gaining confidence to try new community activities.

Our Team

Clinical Team

Our registered nurses hold current AHPRA registration and have experience working with NDIS participants across a range of clinical needs. Many hold additional qualifications in wound care, continence, chronic disease management, and high-intensity supports.

Support Coordination Team

Our support coordinators come from backgrounds in disability, social work, community services, and allied health. They understand the NDIS framework, the local provider landscape, and the practical realities of living with disability in Queensland.

Our NDIS Registration

MediHealth Connect holds full NDIS registration with the NDIS Quality and Safeguards Commission. This means we meet the practice standards required for delivering community nursing and support coordination services, including:

NDIS provider number: 4050126717. You can check our registration on the NDIS provider finder or the NDIS Quality and Safeguards Commission\'s provider register. Our registered groups include community nursing care, high intensity daily personal activities, support coordination and plan management. We do not hold registration for specialist behaviour support, so we implement behaviour support plans rather than write them.

Clinical governance: nursing care is overseen by our registered nursing team. Every nurse holds current AHPRA registration, and support workers who deliver delegated clinical tasks are trained and assessed by a registered nurse before they do so.
