An NDIS support coordinator in the Redlands needs to solve a problem most coordinators never face: getting providers onto the bay islands. The NDIS funds three levels of coordination. Level 1, support connection, is short-term. Level 2, support coordination, is the ongoing service this page describes. Level 3, specialist support coordination, is for participants whose circumstances involve high risk or several systems at once, such as mental health services, housing, child safety or the justice system, and where arrangements have repeatedly broken down.
A specialist support coordinator in the Redlands carries a smaller caseload, works more intensively and is generally expected to bring professional experience of complex cases. Assist life stage transition is a specific NDIS support, and in the Redlands it is where a coordinator earns their keep. Four transitions come up again and again.
The first is the end of school. NDIS school transition support has to start well before Year 12 finishes, so that School Leaver Employment Supports, TAFE or a day program are waiting on the other side. We plan it with the school and the family from Year 10.
The second is moving out. An NDIS transition to independent living on the mainland is a matter of housing, SIL or ILO funding, modifications and a support roster. The fourth is everything else life throws up: a new diagnosis, a carer who cannot go on, a move between the islands and the mainland, a marriage ending. Each is an NDIS life transition, and NDIS transition support exists to hold the plan together while it happens.

Redlands participants have fewer NDIS support coordination providers to choose from than people in Brisbane, so it pays to choose carefully. Three questions do most of the work.
How will they reach you? For island participants especially, find out whether the coordinator visits in person, how often, and what they do when the ferry timetable and a crisis do not line up.
What else does their organisation sell? An independent support coordinator has nothing else to sell you, which is a genuine advantage. MediHealth Connect is not in that position, because we also provide community nursing and other supports in the Redlands. Our answer is transparency: every referral comes with alternatives, no MediHealth Connect service is ever a condition of another, and the reasoning behind each referral is written down. Will they admit when a situation is beyond them? A trustworthy disability support coordinator recognises when a participant needs specialist support coordination and helps them get it, rather than quietly stretching a level 2 service.
